Welcome!- The Survey Service is easy-to-use, confidential, effective and inexpensive. School Districts and ESD Departments use this tool to gather and analyze information from staff, students and parents/guardians.
- If this is your first visit head to the enrollment section first. Complete the information and then select a survey.
- If you have already enrolled head to the survey section. Select the survey to take and then you'll be asked to login. Enter your email address/username and password correctly and then take the survey. If necessary, your password can be sent to you if you have provided a valid email address. To reset your password, enroll again using the same email address.
Notes:
- All contributions are confidential and the results are only displayed in aggregate form. You will see the display with current results after completing a survey. Results for small groups (less than ten) are not displayed
- Please complete all of the surveys that are appropriate for you.
- Do you need assistance with the Survey Service? (This is a PDF file and therefore Adobe Acrobat Reader software is required.)
- Complete the surveys in any order and at any time. It is NOT necessary to complete them all at once.
- It is not necessary to complete the entire survey at one time. You may return and complete the remaining questions at a later time. Your previous responses WILL be displayed when you return.
- It is not recommended to share an email address. Per email address, if you repeat a survey, your record will merely be "updated" and a different record will NOT be created.
- The email address must be valid to send an email "thank you message" after completing each survey..
- Surveys assigned to a email listserv group or ESD 105 school districts match the email address entered at login to the select group listing. If a match is found the survey will be displayed.
- Interested in conducting a Survey Online for your school or district? Get the Survey Order form or the annual District Enrollment form (These are PDF files and therefore Adobe Acrobat Reader software is required.)
